You print out this form and pay by check, money order, or credit card. Use the "Print" command in your browser to print out all pages of this form.  (Note that it will print best with "portrait" orientation.)
Then:

  1. Fill in Section I -- Your Information.    If you're paying by credit card, be sure to fill out that section, too.
  2. Fill in the items you want to order in Section II: Items to Order
  3. Fill inthe "Qty" column for each item you are ordering - enter number of that item you'd like
  4. Fill in the "Amount" column - multiply the Price (each) times the number you entered in the "Qty" column
  5. Add up all entries in the "Amount" column and enter the TOTAL.  (Shipping charges have already been included in the prices.)
